TEAM NOMINATION FORM

Please Note:

1) To secure your team’s position a team nomination fee of $52.00 MUST be paid when submitting the Team Nomination Form.

2) A minimum of 7 x Player's per Team MUST be entered. We highly recommend 8-9 Players for a Season.

3) Please select the appropriate division. Academy teams need to select either Division A or play the age group above. If your team has 4 or more players that played together the previous season please provide the Team name.

4) SCFA reserve the right to move any team they feel has nominated in an incorrect division. An A & B division is on offer in certain age groups but is not guaranteed. It will be dependent on the number of teams that nominate.
5) Only fully completed Team Nomination Forms will be accepted – Teams with only 1-5 players listed will be rejected.
